Claim Form/USA Wrestling - In the past, a member of USA Wrestling, that had sustained an injury would complete two forms, the USA Wrestling Notice of Injury and Health Special Risk claim form. These forms had duplication on what was being requested to report an injury. USA Wrestling's Notice of Injury Form has been eliminated from the process of filing a claim. The HSR claim form will be the only form necessary to report an injury. Anyone can print the Claim Form. The completed form can be faxed to 719-598-9440, e-mailed or mailed to USA Wrestling. Please discard any old forms you may have and only use this form from this point forward. The area asking for a signature of policyholder, title of policyholder and date will be completed by USA Wrestling. If you have questions please let me know.
